Restorative Care - Supporting you to be well for life
Restorative Care supports you to stay active, independent and connected, particularly after a short‑term setback so you can stay well for life. The restorative care pathway focuses on recovery and confidence‑building, helping to prevent the need for increased ongoing support.
A setback may include:
- a fall or injury
- increasing frailty
- dementia or memory changes
- social isolation
At IPC Health, we provide streamlined, coordinated care. You’ll be supported by a dedicated Clinical Care Partner, working closely with our in‑house Allied Health team to develop a personalised plan that reflects our goals, priorities and what being well for life means to you. Wherever possible, we come to you — including providing services in the comfort of your own home.
What to expect when you start with IPC Health
When you choose IPC Health for restorative care:
- A Clinical Care Partner will visit you at home (or at one of our sites if you prefer) to understand what matters most to you and develop a goal‑focused plan
- Our Allied Health team will work with you on your goals over a 12–16 week period
- Your Care Partner will check in with you regularly to review your progress and adjust your supports if needed
I've been approved for Restorative Care - can IPC Health be my provider?
Yes.
- Once your funding has been assigned, there is no waitlist to access restorative care at IPC Health
- Services are available to people living in Melbourne’s western suburbs, including: Brimbank, Wyndham, Hobson’s Bay, Melton, Maribyrnong and Moonee Valley council areas

I'm not sure if I've been assessed
To access restorative care, you’ll need an aged care assessment through My Aged Care.

Services you may be able to access
Your IPC Health Clinical Care Partner will coordinate your services in line with your Support Plan. Supports may include:
- Occupational Therapy
- Physiotherapy
- Podiatry
- Social Work
- Dietetics
- Speech Pathology
Where needed, IPC Health may engage trusted external providers to ensure you receive the right care, at the right time, to support your journey to being well for life.
