Because IPC Health is a community health service, you have access to more than aged care support. Our multidisciplinary team works together to provide coordinated care that supports your health, wellbeing and independence.

As your needs change, your Care Partner can help connect you with a range of services, all through one trusted organisation, including:

1. What is the Support at Home program?

Support at Home is the Australian Government's aged care program that helps older people live independently and safely in their own homes. If you're eligible, you'll receive funding to access services that support your health, wellbeing and daily living.

2. How do I apply for Support at Home?

The first step is to contact My Aged Care and arrange an assessment. If you're approved for Support at Home funding, you can choose IPC Health as your provider. Our Care Partners can guide you through the process and help you get started.

3. Can I change providers?

Yes. You can choose the provider that's right for you and change providers if your needs or preferences change. Our team can help make the transition as smooth as possible.

4. How much does Support at Home cost?

The cost of your services will depend on the level of support you need and the funding you receive. Some services may require a contribution from you. We'll explain all costs clearly and help you understand how your funding can be used before services begin.

5. What services are included?

Support at Home funding can be used for a range of services that help you remain independent and well at home. Depending on your needs, this may include personal care, domestic assistance, social support, nursing, allied health services, transport and other supports that help you stay safe and connected.

6. How long will it take to get started?

Once your Support at Home funding has been approved and assigned, you can contact us to get started. We'll arrange a meeting with your Care Partner, develop a plan based on your goals and organise the services you need as quickly as possible.

7. What's different about our Home Care Partners?

As part of IPC Health, we're more than a home care provider. We offer coordinated access to health, wellbeing and aged care services through one trusted organisation. Your dedicated Care Partner can help connect you with the support you need to stay independent, healthy and Well for Life.

We charge a fee to manage your Support at Home program, which works out to a percentage of your funding. As a not-for-profit health service, our fees are set to provide quality support and personalised care at a cost that’s fair and affordable. See our fee schedule and service price list [PDF] .

The Support at Home program has replaced both Home Care Packages and the Short-Term Restorative Care Programme from 1 November 2025. The Commonwealth Home Support Program will transition to Support at Home no earlier than 1 July 2027.
