How we can help you

Care, comfort and dignity — supporting wellbeing until the end of life

As end‑of‑life approaches, many people wish to remain at home, supported with comfort, dignity and care. Feeling safe, heard and supported is an important part of wellbeing — at every stage of life.

If you, or someone you care for, has three months or less to live and wishes to stay at home, IPC Health can help. Care may include physical support, psychosocial care and pastoral support.

At IPC Health, we take a holistic and person‑centred approach, honouring individual wishes, cultural beliefs and what matters most — supporting comfort and wellbeing, right through to the end of life.


What to expect when you start with IPC Health:

  • A Care Partner will visit you at home to understand your needs, preferences and priorities
  • Your Care Partner will work with you to put supports in place and will stay in regular contact to monitor your services and changing needs


I've been approved for end-of-life funding - can IPC Health support me?

Yes.

  • There is no waitlist once your funding has been assigned
  • Available to people living in Melbourne’s western suburbs, including: Brimbank, Wyndham, Hobson’s Bay, Melton, Maribyrnong and Moonee Valley council areas

    To access end‑of‑life funding, an aged care assessment through My Aged Care is required.

      People aged over 65 years and Aboriginal and Torres Strait Islander people 50 years and over living in Melbourne’s western suburbs, including: Brimbank, Wyndham, Hobson’s Bay, Melton, Maribyrnong and Moonee Valley council areas.
