What we can help with
Make positive health and wellbeing changes with tailored support by our Wellbeing Coordinators.
We make it easier for you to access physical, mental and social health services by working with you to plan your care, build healthy habits, and focus on what matters most to you and your goals.
Your Wellbeing Coordinator works alongside you to understand your goals and connect you with the support you need.
They can help you:
- Find the right services to support your health and wellbeing goals
- Navigate the health care system
- Overcome barriers to getting care
- Build confidence to make positive, lasting changes
Types of support
Wellbeing Coordinators support you with:
- Health and wellbeing needs
- Goal setting and staying motivated
- Connecting you to housing, money or food support services
- Linking you with transport services and social connections
We also provide support and information on:
- Healthy eating and physical activity
- Quitting smoking or vaping
- Better sleep and stress management
- Managing long-term health conditions
The team offers one to one personalised coaching as well as small groups so you can make connections and learn with likeminded people.
The Living Well team runs the following groups:
- Learn to Relax: learn to effectively manage stress and different ways to relax
- Live Well for Life: for people who have diabetes and heart disease (or those at risk of getting these conditions) to learn how to improve their health
Waiting list times
You may have to join a waitlist or need to wait until a new group starts. Call us or ask online to register your interest.
