You have the right to request access to your personal health information and ask for corrections if you need to.

This form is also available at any IPC Health campus, just ask our friendly Client Services staff. 

An administration fee may apply for copying or sending information, in line with the Victorian Health Records Act 2001. Learn more about the Act here . 

If you submit your request electronically, you’ll need to provide a certified copy of your identification before records can be released. If you apply in person, you can show your identification when submitting the form.

For access to the health records of a deceased person, please complete the same form and provide evidence of your relationship.
