What we can help with

Seniors Lifestyle Victoria is a free program that helps older Victorians to stay active, stay strong and stay connected.

We can support you to be well for life in your senior years. To help you get back to the things you used to be able to do.

Are you finding it harder than before to get around or do everyday things like shopping, cooking, cleaning or other activities you used to enjoy?

Would you like to join a program that helps you stay active, healthy and independent at home, with regular check-ins and personalised help with things like diet, exercise and social connection?

We help you navigate the healthcare system and connect you with local services and activities that matter most to your wellbeing and independence.

    Types of support

    You will receive six months of support from a wellbeing coordinator or wellbeing navigator. They will help you to identify your health and lifestyle goals to improve your quality of life, connect you with services, and check in regularly to make sure your plan is working for you.

    The program offers a range of services to support you to stay independent and well. This might include support services focused on:

    • physical activity
    • falls prevention and strength building activities
    • nutrition and healthy eating support
    • medication management
    • social connection and wellbeing

    Using this service

    The Seniors Lifestyle Victoria program is an early support program for older people who are generally in good health but need a bit of help to stay active, healthy and independent.

    IPC Health offers this program in the local government areas of Brimbank, Wyndham, Hobsons Bay and Melton for:

    • people aged 65 years and older, or 50 years and older for Aboriginal and Torres Strait Islander people
    • people living in the community (not in aged care)
    • people with mild to moderate changes in health who would like support to improve their health and wellbeing.


    It could be that you have experienced a recent fall or hospitalisation, have been losing weight without trying or finding it difficult to maintain your independence and engagement in everyday life.
