Our Case Management Services and Options
Consumer Managed
This level of support is beneficial where you prefer to coordinate your services and would like to continue to do so independently with our help for initial service set up, budgeting and monthly check in phone calls.
Partial Consumer and IPC Health Managed
Beneficial option for where you would like additional support to coordinate your direct services, but would like to maintain independence with coordinating health referrals and advocacy.
IPC Health assists with direct service coordination, budgeting and visit you on a quarterly basis to assist with care planning.
IPC Health Managed
This option is optimal where you prefer for all service coordination to be managed by IPC Health. It gives you and your family peace of mind, knowing that all care and budgeting is planned with you but is completed by us. There are unlimited phone calls and regular home visits included to support your care needs.
