Community Access West is an Outreach Program that supports adults with an Acquired Brain Injury to explore and enjoy their community through social and recreational groups and activities.
To be able to use this service you must:
• have evidence of an Acquired Brain Injury (not including degenerative neurological disorders)
• live in the Western Metropolitan region of Melbourne and
• be aged 18 to 65 years
We also accept ISP funded individuals and are a TAC approved provider. Eligibility criteria for TAC and ISP individuals may vary from above.
How do I make a referral?
Referrals can be made by contacting the Acquired Brain Injury – Community Access West Intake Worker: 03 9216 7815.
Referrals can be made by the person with the injury, a family member, a carer or a professional. If you are a TAC funded individual, request a referral via your Support Coordinator.
Is there a fee?
Yes. Please speak to the intake worker about what fees may apply.
Can I have an interpreter?
Yes. Ask if you need an interpreter. Interpreters are free.
