How we can help you

What we can help with

Finding out what services are available to help you live independently at home can be hard and the assessment processes to see if you’re eligible can also be confusing.

The IPC Health’s Care Finder Service helps older people without close support find the right aged care services. We can also get you involved in the community so you can meet new people and participate in activities you enjoy.

Our care finders will come to you to talk about your needs, what options are available, how to apply and how to prepare for any assessments you might have to do. We’ll be on hand once your services are up and running and can also help you change services if they’re not quite right.

    Using this service

    People aged over 65 years and Aboriginal and Torres Strait Islander people 50 years and over, as well as people experiencing homelessness or who are at risk of homelessness who are eligible for aged care service and live in Brimbank, Hobsons Bay, Maribyrnong, Melton, Moorabool and Wyndham.
